Please Note: Where a form is listed as 2 pages, a link to the second page is at the bottom of the first page. As the forms do not contain a navigation bar, you may need to use your browsers "Back" button when you have finished. Select Print... or Print Frame... from your browsers 'File' menu to print the form. To insure that the whole page has printed, look for "END OF PAGE" or "END OF FORM" at the bottom, and check the margins for any cut off words or undesirable linefeeds. Refer to the Tips section below if you have any problems.

Tips on printing the Internet Forms:
Certain browser settings can affect the final appearance of the printed forms. If your printed copy does not look right i.e.- extra linefeeds causing broken lines, form cut of at bottom etc. you may need to temporarily adjust one or more settings on your browser. First try reducing the size of the screen font. On Netscape 3.0®, from the 'Options' menu, select 'General Preferences'. Click the 'Fonts' tab and select a point size for the 'proportional' font. A 12 point font should be good. On MS Internet Explorer®, from the 'View' menu, select 'Fonts'-'Medium' or 'Small'. If these settings are already the selected defaults and the printed form still needs adjustments, check the margin settings from 'Page Setup...' on the 'File' menu. A setting of .500 all around is usually the best. Also, try turning off the 'Header' and 'Footer' information the browser attaches to the page. This may be a must if you are using an tractor fed printer. Play around a bit. You may decide to keep some of the new settings.
